Accepting a spot in our program requires dedication and commitment to your development as a student golfer and ambassador of the foundation. All selected recipients must agree to the following:

Golf Instruction

1. Attendance must be consistent.  If you miss more than 10% of your lessons without good cause, then you will be at risk of losing your spot.
   
2. You must participate in a minimum of three tournaments per quarter. High school participants must compete in two-day tournaments.
   
3. Applicants age 13 and older must maintain an Instagram and/or Facebook golf page and tag DEF in all golf-related posts. A minimum of one golf post per week, plus a post after each tournament is required. For applicants under 13, a parent/guardian must agree to create and manage social media pages. A strong online presence is essential for NIL opportunities and College recruitment.
4. You must represent yourself and the foundation in a positive and respectful manner, both on and off the course.

Required Documents
All required documents, including applicable tax documentation, a current or most recent report card, and letters of recommendation (required for applicants ages 14 and older only), must be uploaded when your application is submitted.

Please attach the following items:

☐ Proof of household income or financial documentation (one of the following)
Most recent federal tax return (Form 1040 — first page only)
W-2 or 1099
SSI/Disability Benefit Statement
SNAP/Medicaid Eligibility Letter
Unemployment Documentation
Other reasonable proof of household income
All financial information and supporting documentation will be kept confidential and used solely for program eligibility and review purposes.

Please attach the following items:
☐ Two letters of recommendation
Provide two letters of recommendation from an adult familiar with the applicant’s character and development (coach, teacher, mentor, pastor), who can provide general insight into the applicant’s work ethic, sportsmanship, academic effort, commitment to golf, and/or overall potential for growth and success.
